Dear Parents/Guardians:
Welcome to the 2024-2025 school year! Many of the forms that are required, such as physical form, medication administration forms, EpiPen administration form etc, are all available on my staff page on the school website. Any child that requires medication must have the medication administration form filled out and signed by the child’s physician and the parent. Every school year requires new forms for medication administration, so if your child had medication in the Nurse’s office previously, they are required to obtain new forms for this school year in order for the nurse or her delegate to administer them. Please send your child’s medication in the original container with identifiable factors such as name and date of birth, as well as expiration date.

Students in Pre-k 3 and Pre-K 4 are required to have their flu vaccines completed by the end of November when the school’s health records are audited.
Kindergarten students are required to have physicals completed as well as their second MMR vaccine. Any child who is deficient in vaccines or physicals will not be allowed to return after Christmas break until they are compliant. Religious exemptions are not accepted by Sacred Heart School or the Archdioses.
New students are required to have physicals as well.
Any child who is participating in a school sport must have a sports physical completed by their physician in order to participate. This is required by both the Department of Health and the Department of Health and must be followed. The form will be on my staff page; Mrs. Durocher will also send it out in an email. Please return the first page of the form only to the school.
